ABOUT
Asthma Bridge is a simple tool for physicians, school nurses, and other health professionals to request community-based support services for children with asthma. Asthma Bridge directs referrals to an approved network of certified and trained professionals who deliver evidence-based home environment assessment and/or self-management education services. This closed-loop referral management provides feedback to the referring individual.

- Improve access to essential community-based services known to be effective for management of childhood asthma.
- Coordinate delivery of environment and education support services in collaboration with payers
- Support evaluation of outcomes related to environment and education support services
