ABOUT Asthma Bridge is a simple tool for physicians, school nurses, and other health professionals to request community-based support services for children with asthma. Asthma Bridge directs referrals to an approved network of certified and trained professionals who deliver evidence-based home environment assessment and/or self-management education services. This closed-loop referral management provides feedback to the referring individual.

GOALS
Improve access to essential community-based services known to be effective for management of childhood asthma.
Coordinate delivery of environment and education support services in collaboration with payers
Support evaluation of outcomes related to environment and education support services
