Physicians, school nurses, and other health professionals request community-based support services for children with asthma through Asthma Bridge website or phone number (573) 651-2355. Available services:
Home Environment Assessment: Families receive a series of home visits by a trained environmental specialist who assesses the home for triggers and provides improvement recommendations.
Step 2: Appointment Scheduling
The Asthma Bridge team directs referrals to a certified and trained professional who is available to provide home environment assessment services. This provider will contact the family directly to schedule an appointment. Families will receive name, photo and contact information of their provider when the appointment is scheduled. The Asthma Bridge team will handle any special approvals that may be required by a health insurance plan.
Step 3: Service and Support
One day prior to scheduled appointment, the time and place will be confirmed by phone call and/or text message. The provider will arrive at the appointment on-time. Follow-up appointments or referrals for additional support services may be made at that time.
Step 4: Follow-up with Healthcare Team
A few days after the appointment, the home assessor and/or health educator will send a report to the child's health care team, such as a physician, nurse practitioner, or school nurse, Reports are sent only with permission of the parent or guardian.
Step 5: Quality and Satisfaction Check
A few days after an appointment, the family will be contacted by the Asthma Bridge team to assure services meet expectations and ask for other feedback.
