Home Environment Assessments

Home environment assessments are provided by individuals and organizations who have received specialized training and are credentialed by the Southeast Missouri State University Center for Environmental Analysis.
Asthma Bridge coordinates referrals based on location of home and service provider availability. Service Providers (as of 09/27/2018)
A listing of service providers credentialed by MO Health Net Fee For Service is available here. Search "ALL Provider Types" for "Asthma In-home Environmental Assessment". Service providers available for families without insurance or are members of health plans offered by managed Medicaid health plans and commercial insurance companies may have a different network of providers available to them. Asthma Bridge considers health insurance type when coordinating referrals and makes sure families without health insurance receive needed services. |

Self-management Education

Self-management education services providers have earned certification through the University of Missouri School of Medicine Asthma Ready Program. Each providers uses an evidenced-based curriculum and materials for teaching children and families the fundamentals of asthma care management. Appointments are generally held in a suitable community setting selected by the client and provider, such as a home, school, library/community center, public health department office, etc. MoHealthNet (the Missouri Medicaid) program provides reimbursement for services when delivered in the home setting.
